Our Continuum of Care program is designed to enhance our residents lives by providing continuity in care from people that know them and care about their future.  We provide a skilled resource to assist in navigating the complexities of healthcare changes. We have support options available so, if and when, additional help is needed you can add that help to maintain an independent living lifestyle.

The goal of the program is to support an ‘aging in place’ philosophy.  We provide resources and tools for you and, depending on the need, those resources could be in your current home or in a Lang Nelson assisted living community that meets your needs and exceeds your expectations; and that we would help you transition to.
